Some publishers accept unsolicited manuscripts - some don't. Check with the publisher before you send your manuscript.
A literary agent will read and assess your work and, if it is accepted, will negotiate on your behalf with publishers and look after your commercial interests.

Advice
Each agent has their own interests and terms, and some will charge a reading fee. Most agents prefer a preliminary letter outlining the work, so don't just send off your manuscript without contacting the agency first.
- Make it easy for the agent - a clean, well-laid out typescript is essential
- Use A4 paper
- Use unjustified double spacing with wide margins
- Number all pages
- Include your name and address on a title page
- Pages should be unbound and unstapled
- Don't send your only copy
- Always include a stamped, self-addressed envelope
- Only send your work to one agent at a time
